MnDOT is collecting personal information about you on the Youth Leadership application form. Some of this information is classified as private under the Minnesota Government Data Practices Act. The information on the application form will be used by MnDOT staff to select applicants and to contact you about your potential participation in the program. Information in the form will not be used for any other purposes. Your private data will only be accessible to MnDOT employees whose job reasonably requires access, to anyone you specifically authorize to receive the information, pursuant to court order, and by any other person or entity authorized by state or federal law. If you are a minor, you have the right to request that parental access to your private data be denied. You are not required to provide the information on this form and there are no consequences if you decline to do so; however, if you do not provide it, we will have no method of contacting you to participate in the program.
